fix: add env var fallback to backend-size in both backends
ioctl on stdout's fd can return 80x24 even when the terminal is larger. Add COLUMNS/LINES from the shell as a fallback. Also adds ioctl-based sizing to simple-backend (was hardcoded 80x24).
